Meet Carmen Colón, APRN

Carmen Colón is part of a movement that is restoring the physician/patient relationship by providing comprehensive care through a focus on prevention and wellness. She enjoys working with both children and adults of all ages.

Carmen is a Board-Certified in Family Practice, completing her training through the University of Cincinnati. In addition to experience in primary care, she has been a member of TMH trauma team, where not only did she scrub into surgery, but she also managed her patients’ medical needs in the intensive care unit. Most recently, she practiced in the Tallahassee Pulmonary Clinic where she managed complex lung and sleep disorders.

Carmen has a special interest in Functional Medicine. When not practicing medicine, you will find her enjoying a good book, watching a movie, or crafting.

Membership: Join a Practice that is Personal and Caring

Instead of billing insurance companies for each visit, our members pay a monthly membership fee "directly" to us, which covers all visits and communication! This allows us to focus on patients and not paperwork.

Included in your membership is an annual, comprehensive medical examination. We will review all your current medical issues, make sure that they have been properly evaluated, and formulate a plan to address any medical issues. She will also make sure you have had the approrpriate screening laboratory tests and imaging.
Includes acute visits for common complaints, such as cough, congestion, sore throat, aches, pains, and injuries. Also visits concerning emotional or gynecological concerns.
We use technology for convenience and peace of mind. Text your provider with a question, because maybe a visit is needed, but maybe not. Forgot to ask a question? Just send a quick text. Need a form completed? Email it. Done. Easy.
Common everyday rapid laboratory tests including strep, RSV, flu, and mono testing are availabe for $5. If more extensive lab testing is necessary, you can bill your insurance or most common lab tests are available for about $20 or less. We can usually draw your blood at Access Direct Primary Care.

Tired of explaining the issue over and over? Or having different doctors switching between different treatments? At Access Direct Primary Care, we are a team who works together with the same plan.
Sometimes a medical problem requires the help of an specialist or therapist. Access Direct Primary Care will work with you to coordinate the care between specialists.
Routine vaccinations are very important! Our vaccinations are covered by private insurances, such as CHP and Florirda Blue, or Medicaid. We can also provide childhood vacciness free-of-charge to uninsured children.

GAHT Sent To Your Pharmacy

We send GAHT medications, such as estrogen, testosterone, and spironolactone to your pharmacy of choice.

We have availability for patients to start or continue Gender Affirming Hormone Therapy (GAHT) in days, not months.

Access Direct Primary Care is an out-of-network provider and does not participate in any insurance plans. Reimbursement for out-of-network providers varies significantly from plan to plan. Please consult with a representative from your insurance company for your policy's details.
Membership fees are charged monthly to your bank account, credit or debit card. We do not accept cash for memberships.
Yes. Access Direct Primary Care is not a substitute for health insurance, but you be able to save money by switching to a high-deductible plan or a cost-sharing plan. Most health insurance plans cover at least some the costs of scans and lab tests just as they would if ordered by an in-network physician. Please consult with a representative from your insurance company for your policy's details.
Call our office to set up a complimentary meet and greet with us to discuss the upcoming arrival of your newest family member. We will see your baby one to three days after discharge from the hospital.
In Tallahassee, both TMH and Capital Regional Medical Center have hospitalists that management hospital admission. We will work closely with the TMH or CRMC hospitalists to optimize your care, and we will provide seamless follow-up upon discharge.
Yes. Members, when appropriate, can use photos, phone calls and video calls to communicate with us. In many cases questions can be answered and diagnoses can be made (or ruled out) without an office visit. This saves families time and money, and it is covered by your membership, free of charge.
Given the small size of our practice, we will have an excellent understanding of your family's medical history. So we will do as much as we safely can via phone, text and video. This long-distance care may include calling in a prescription to a local pharmacy or helping you find a local urgent care or ER should further evaluation or testing be required.
We are advocates of vaccines, and we feel strongly that administering them based on the American Academy of Pediatrics (AAP) recommended schedule is one of the most important preventative care measures that any physician can offer. Immunizations have saved more lives than any other medical advancement. At the same time, we understand the fear many families have concerning vaccines. We will accept families that wish to delay or forgo vaccines, as long as they are willing to have an open discussion about vaccines. In most cases, we can create a vaccination plan that parents are happy with.
